Who Can Complete a Form 888?
According to Form 888 (Design date July 2023)
A person who completes the Form 888 must:
- know the visa applicant and their partner or fiancé(e) and the
history of their relationship; - is at least 18 years of age.
The person completing this form must provide documentary evidence of their current name, age and, where applicable, Australian citizenship or Australian permanent residency (for example, a copy of their birth certificate, Australian passport or passport containing a permanent visa).

Form 888 is a Statutory declaration by a supporting witness in relation to a Partner or Prospective Marriage visa application.
Form 888 must be completed by a person who knows the visa applicant, and their partner, and the history of their relationship.
The person completing form 888 must be an Australian citizen or Australian permanent resident, who is at least 18 years of age.
If the visa applicant is outside Australia and does not have an Australian citizen or Australian permanent resident available to complete a form 888, any person who knows the applicant is able to complete the form.
The form 888 (Design date 10/20) states: The person completing this form must provide evidence of their current name, age and, where applicable, Australian citizenship
or Australian permanent residency (for example, a certified copy of the birth certificate, Australian passport or passport containing a permanent visa). All copies must be certified.
As passports no longer contain a permanent visa stamp or document, the above instruction does seem outdated.
